Job Summary

The Nurse Practitioner will provide primary care to a panel of HIV positive patients, coordinating care with existing HIV providers in SSHARC clinics and with specialty care at Oak Forest Ambulatory Care Center and the CORE Center.  The Nurse Practitioner will also assess SSHARC patients for substance abuse and mental health problems and coordinate care with appropriate providers in the south suburbs.  The Nurse Practitioner will work collaboratively with the Clinical Pharmacist on adherence issues.  Finally, the Nurse Practitioner will direct the program’s quality assurance efforts, working with the Data Manager on data collection and analysis.  

Responsibilities

  • Provides independent primary care for HIV positive patients as delineated in the collaborative agreement.
  • Collaborates with the health care team and SSHARC staff to provide quality care to patients.
  • Directs and evaluates the effectiveness of patient care i.e. develops patient education materials and quality improvement initiatives.
  • Makes an assessment of patients’ substance abuse and mental health status.
  • Coordinates referrals for medical subspecialty and behavioral health care
  • Maintains positive relationships with CORE Center staff and Oak Forest Ambulatory Care Center staff.
  • Completes reports related to patient care activities and Title III objectives
  • Participates in multidisciplinary staffings

Qualifications

Current Illinois licensure as a Registered Nurse and as an Advanced Practice Nurse; master’s degree in Nursing from an accredited school of nursing; progressive experience and advanced knowledge in HIV primary care and behavioral health; certification by a nationally recognized body as designated by the rules and regulations of the Nurse Practice Act.
