Job Summary

Responsible for the day-to-day operations, and clinical oversight of the Care and Retention program.  Monitors progress toward meeting program objectives, develops programmatic materials, and confers with the Ryan White/Retention Services Manager and the Director of Community HIV Primary Care.  Hires and supervises patient navigator/outreach staff, and prepares and submits program progress reports to the Ryan White/Retention Services Manager.

Responsibilities

  • Hire, train, and provide clinical supervision for patient navigator/outreach staff.
  • Develop and implement program protocols, in collaboration with the Ryan White/Retention Services Manager.
  • Coordinate and implement the project goals and objectives.  Provide clinical programmatic direction.
  • Develop and review assessment tools, data collection tools, and program materials in collaboration with the Ryan White/Retention Services Manager, and the AIDS Foundation of Chicago (AFC).
  • Participate in organizational care/retention meetings, as requested.
  • Participate in NAF partner meetings sponsored by AFC.
  • Promote and explain the program, and its objectives, in collaboration with the Ryan White/Retention Services Manager, to the clinical teams and to external community partners.
  • Establish and maintain collaborative professional working relationships across various RMRCC programs, departments, and disciplines.
  • Co-represent the program at local and national meetings.
  • Design, develop and implement quality improvement activities to improve and enhance program operations.
  • Assist with IRB application process and on-going IRB communication.
  • Monitor data collection.  Prepare and submit program reports as required.

Qualifications

Qualifications – Knowledge/Skills/Experience:

  • B.S. in nursing preferred; or health related field with 3-5 years experience in clinical setting. A minimum of five years experience in the HIV health care field. Strong written and verbal communication skills. Experience in program monitoring. Supervisory experience. Familiarity with medical terminology.
  • Experience working in clinical setting with multi-disciplinary team.
