Job Summary

The Peer Patient Navigator will conduct outreach to HIV-positive men who have sex with men (MSM), who have not engaged in health care services, or who are lost to care.  Peer Patient Navigators will develop professional relationships with MSM patients receiving HIV primary care at the CORE Center and will assist patients in follow-up and execution of case management and medical treatment plans, address barriers to care, and connecting patients to appropriate staff to obtain public benefits and entitlements.  The Peer Patient Navigator may conduct home visits to assist patients as necessary for the retention of care. 

 

Responsibilities

  • Initiates immediate contact with newly HIV diagnosed MSMs, on the Stroger Hospital campus, and through the National AIDS Foundation (NAF) Chicago-based network, and the Cook County HIV Primary Care Consortium.  Establish rapport and assist with linkage to care at the CORE Center.
  • Reminds patients of their appointments and assist with addressing barriers to care.
  • Accompanies patients through their first visit and connects with the New Patient Orientation at the CORE Center to orientate them to CORE Center services and how to access them. 
  • Follow-up to assist patients in accessing their second and subsequent visits to the CORE Center for medical and supportive services.
  • Assist patients in understanding their case management and treatment plans and the importance of patient’s follow-through.
  • Communicates with the multidisciplinary care team about individual patient needs and concerns.
  • Remind patients to bring requested and/or other necessary documentation for medical visits and supportive services.
  • Arranges for transportation to medical and other appointments.
  • Shares knowledge and education to improve patient’s knowledge and attitudes toward HIV and necessary services.
  • Conducts professional home visits as necessary in consultation with the interdisciplinary team to provide peer support and to assist with overcoming barriers. 
  • Enters data into the program’s database.
  • Participates in all administrative and staff development meetings.

Qualifications

QUALIFICATIONS – KNOWLEDGE/SKILLS/EXPERIENCE:

  • Minimum of a high school diploma or equivalent.
  • Knowledge of HIV treatment and prevention.

Preferred Skills/Qualifications:

CORE Center consumer for at least one year; and graduate of CORE Center Peer Training Program.
