JOB SUMMARY:
The Advanced Practice Provider (APP), under the discretion of the Cook County Health Integrated Program (CCHIP) Program Director and under the direct supervision of the Provident HIV Program Attending Physician, provides care to persons living with HIV (PLWH) and persons at risk for HIV at the Provident Hospital of Cook County and other CCHIP ACHN locations as needed.
JOB DUTIES and RESPONSIBILITIES:
- Provides primary care services to PLWH in two to six clinic morning and afternoon sessions per week, including: appointments, telephone follow-up, patient interviews, health maintenance counseling, compliance/exit interviews, liaison with diagnostic services, patient advocacy, and referrals to case managers and chemical dependency counselors as warranted
- Provides STI assessments, evaluation and treatment as well as Pre-Exposure prophylaxis (PrEP) to those at risk for HIV acquisition
- Counsels patients regarding transmission of STIs/HIV and risk reduction strategies
- Provides evaluation, treatment, and counseling for viral hepatitis (hepatitis B and hepatitis C) patients in collaboration with the attending physician and assists with obtaining prior authorization for said services, if and when needed
- Elicits a comprehensive, accurate history and performs a thorough routine physical examination of patients who are 18 years of age and older on initial presentation to clinic
- Elicits an accurate history and performs a problem-oriented screening examination for patients in follow-up situations
- Selects performance and/or interprets the appropriate routine laboratory and diagnostic studies/procedures
- Maintains records of routine laboratory and diagnostic studies/procedures for the purpose of completing medical records and maintaining an adequate database
- Identifies problems, organizes and integrates patient information, and records and presents pertinent data
- Maintains problem and medication list and documents well in the electronic health record (EHR)
- Initiates/implements a medical management plan in collaboration with the supervising physician
- Instructs and counsels patients regarding physical and mental health to include diet, health maintenance, therapy, referral, and follow-up services
- Reviews and monitors medical management and therapy plans and implements appropriate continuing treatment in collaboration with supervising physician
- Cooperates with support services personnel to implement the social service treatment plan in conjunction with the interdisciplinary team
- Attends regular monthly Provident HIV Program, Provident Hospital, Provident HIV Program Quality Improvement and CCHIP Domain meetings
- Attends daily multidisciplinary case conference meetings
QUALIFICATIONS:
- Graduate from a Physician Assistant (PA) program approved by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or Advanced Practice Nurse (APRN) program approved by Commission on Collegiate Nursing Education (CCNE) required
- Board certified/board eligible either by National Commission on Certification of Physician Assistants (NCCPA) or American Nurses Credentialing Center (ANCC) and able to practice in the state of Illinois required
- Master’s Degree with two or more years of HIV/AIDS care desirable
- Participation in continuing education in HIV to remain current
- Must maintain strict confidentiality regarding patient information
- Must perform duties in keeping with CCHIP policies and procedures
- Experience with multiple diagnosed patients (concurrent mental health and substance abuse problems) a plus
