HIV Health Education/Discharge Planner

JOB SUMMARY:

Function: Works closely with the HIV Manager to assure that the project scopes and objectives are achieved. The primary responsibility of this position is to help ensure that all high-risk detainees are educated on HIV and the identified HIV positive detainees are linked to an external case manager for community services that provide comprehensive HIV and social services. 20 hours a week

Job responsibilities include, but are not limited to:

• Conducts client intake, periodic assessment, and care planning for clients
• Attend monthly and quarterly meetings (Zoom in person TBD)
• Educate detainees on HIV, STD, Harm Reduction
• Prepare monthly and quarterly reports
• Adhere to the agency compliance with HIPPA, Contracts, other regulatory bodies, confidentiality policies, etc.
• Attend the Continuity of Care weekly with external Case Manager
• Other duties as assigned

Qualifications/ Basic job Requirements

• At least two (2) years of history in HIV and STI/STD care environment and some discharge planning experience highly preferred
• Excellent oral, written AND COMPUTER SKILLS
• Certified HIV Counseling/Tester
• Must be organized and able to work in a high volume setting and a motivated self-starter
